Target Case Management
This service is available to
clients who are identified as needing help with being linked to
community services such as: accessing medical, social, or educational
services, housing, cash assistance, court appearances, disability,
SSI, or other services essential to meet basic human needs, while
remaining independent in the community. We work with Children 2-17
and adults 18 and up and their families.
Recipients are assigned to case managers supervised by well prepared and experienced staff.
Psychosocial Rehabilitation Services (PSR)
This service is a classroom based recovery program which offers services for
individuals (both school aged children and adults) with mental
health diagnosis as well as substance abuse. The goal of this
is to assist the individual with eliminating functional deficits
both interpersonal and environmental created by their disabilities.
Additionally, this program is designed to restore social skills
for independent living and effective life management. A variety of
topics are covered on these sessions, including:
Individual Therapy
This is available to individuals who are experiencing difficulties
which has impaired their ability to effectively handle life’s
challenges. You will work one on one with a master’s level or
licensed professional focusing on identified treatment goals.
Therapy typically consists of 1 hour sessions per week.
Family Therapy
Family Therapy is similar to individual counseling except the session
includes the family members with goals indentified for all. Generally,
family therapy is usually provided when the person served is a
child/adolescent. Family Therapy usually takes place for 1 hour per week.
Group Therapy
Certain issues are better treated in a group setting. Believe and Trust offers Group Therapy for various issues and populations
based on the client’s needs.
Psychiatry
Psychiatric Services are available to individuals needing an evaluation
and monitoring for psychotropic medications. Psychiatric services are
provided by psychiatrists and APRN Staff. We work with ages 2 years old
and up (if 2 years old, child must display good verbal skills).
Evaluations and Assessments
Evaluation of an individual's health that considers biological, psychological, and social factors.
Transportation
We offer complementary transportation for services rendered in an in-person setting such as Psychiatry, Psychosocial Rehabilitation, and Individual Therapy.
